1. Scope of Services

Hubur Enterprises LLC (“Hubur”) offers professional Revenue Cycle Management (RCM) services including but not limited to:
● Medical Billing & Coding
● Provider Credentialing
● Prior Authorizations & Eligibility Verification
● Claims Submission and Follow-up
● Patient Statements & Collections
● Aging Claims (A/R) Recovery
● Healthcare Virtual Assistance
● Reporting & Analytics
● Worker’s Compensation
All services are performed in compliance with HIPAA and other applicable regulations. 

2. Service Delivery Policy

Service timelines are based on information and documentation provided by the client. Hubur is not responsible for delays resulting from incomplete, incorrect, or late submission of required materials.
Time-sensitive requests (e.g., payer deadlines) are handled with priority but cannot be guaranteed unless pre-arranged and confirmed in writing. 

3. Payment Terms

Payment for services is governed by individual service agreements. Unless otherwise stated:
● Billing services are charged at a percentage of the monthly collected revenue.
● Credentialing and project-based services are billed per milestone or flat rate.
● All payments are due within [05–10] days of the invoice date.
Accepted payment methods include ACH, Zelle, PayPal, and Wire Transfer.

4. Refunds & Cancellations

Due to the nature of our services, refunds are not issued once work has commenced. Cancellation requests must be made in writing with a 30-day notice. Project-specific terms may apply per contract. 

5. Client Responsibilities

Clients agree to:
● Provide accurate and timely information
● Respond to communication requests
● Grant necessary system access
● Adhere to agreed service timelines 

6. Confidentiality & Compliance

Hubur will maintain the confidentiality of all client and patient information, in accordance with HIPAA and relevant privacy laws. Data is stored and accessed securely, and access is limited to essential personnel.  

7. Intellectual Property & Content

Training materials, SOPs, and documents shared by Hubur remain the intellectual property of Hubur Enterprises LLC. Unauthorized use or distribution is prohibited. 

8. Communication Policy

By engaging with Hubur, you consent to receive updates and notifications via phone, email, or SMS. To stop receiving texts, reply STOP. Standard message and data rates may apply.  

Customers will send START to (925) 471-8100 to receive text messages from Hubur LLC. Customers agree to receive informational messages (appointment reminders, account notifications, etc.) from Hubur LLC. Message frequency varies. Message and data rates may apply. For help, reply HELP or email us at contact@huburllc.com. Customers can opt out at any time by replying STOP.

9. Dispute Resolution

Any disputes shall be resolved through mediation or arbitration in accordance with the laws of Texas and/or California, depending on the jurisdiction of service.  

10. Legal & Miscellaneous

● These Terms may be updated from time to time.
● If any provision is found unenforceable, the remaining provisions remain valid.
● The Terms are governed by the laws of the State of Texas.

Terms and Conditions (SMS Communication)

1. Introduction

Welcome to Hubur Enterprises LLC (“Hubur”). By accessing or using our services, including receiving SMS communications, you agree to comply with and be bound by these Terms and Conditions. If you do not agree with these terms, please do not engage with our services.

2. Consent for SMS Communication

By providing your consent to receive SMS communications, you acknowledge and agree to receive text messages from Hubur Enterprises LLC (“Hubur”) at the phone number you provide. Information obtained as part of the SMS consent process will not be shared with third parties.

3. Types of SMS Communications

If you have consented to receive text messages, you may receive SMS communications related to the following:
● Customers & Guests: Appointment reminders; service updates for billing/RCM, credentialing/enrollment, prior‑auth status, and WCAB checkpoints; document/signature requests; invoice/payment notices; policy or system alerts. (No PHI via SMS.)
● Job Applicants: Application status updates; interview scheduling/confirmations; pre‑hire paperwork and background‑check reminders; onboarding, start‑date, and training notices.

4. Standard Messaging Disclosures

● Message Frequency: Frequency of messages may vary depending on your interactions with us.
● Standard messaging rates will be charged by your mobile service provider.
● You can opt-out of receiving SMS messages at any time by texting “STOP” to the number from which you received the message.
